Inkoverlay PDF Forms turns any PDF form into a web form. Upload the PDF, auto-detect its fields (or add them manually), calibrate their exact position visually on the rendered page, and publish. When someone submits the web form, their answers are overlaid back onto the original PDF at the calibrated coordinates — text lands in the real blanks, an X lands in the real checkboxes — and the filled PDF is emailed as an attachment to both the site admin and the person who submitted it (if they gave an email).
Features
Visual calibration editor — upload a PDF, auto-detect text fields, Yes/No checkboxes, and single checkboxes from the PDF’s own text layout, then drag/fine-tune each one directly on the rendered page image.
Exact PDF overlay on submission — answers are written back onto images of the original PDF pages at their calibrated coordinates, so the emailed PDF looks like the form was filled out by hand.
Drag-to-reorder pages in the editor.
Bilingual (Persian/English) forms — optional per-field Persian label with an RTL layout and a language toggle on the public form, plus a separate English/Persian toggle for the admin editor screens themselves.
Submission log in wp-admin for diagnosing mail delivery, with per-submission PDF download and delete.
Honeypot spam protection, no external services required.
Style presets and primary/button color pickers, no custom CSS needed.
Requirements

PHP with the GD or equivalent image extension (bundled TCPDF handles PDF generation)
