Contact Form 7 is a really powerful and one of the most popular form builders for WordPress. It can manage multiple contact forms, plus you can customize the form and the mail contents flexibly with simple markup.
Using the SMS Integration plugin you can send SMS notifications to admin or any number whenever someone submits your forms – you get a nice text message, wherever you are. The message text can be customized with form data shortcodes. If you’ve got multiple forms, you can setup a different mobile number for each of them
